Health Requirements

St. Leo’s School requires that each student have a physical examination upon entry into school; any modifications required for full participation in the school program should be stated at that time. The physical must be performed by the child’s health care provider no more than 365 days prior to school entry.

Immunization Records

According to the New Jersey State Department of Health, no child may be admitted into school without proper verification of the following immunizations:

DTaP – (Age 1-6 years): 4 doses, with one dose given on or after the fourth birthday OR any 5 doses. (Age 7 or older); 3 doses of Td or any previously administered combination of DTP, DTaP, and DT to equal 3 doses.

Tdap: One dose is required for children entering grade 6 and all students transferring into a New Jersey school on or after 9-1-08 and born after 1-1-97.

IPV (Polio) – (Age 1-6 years): 3 doses, with one dose given on or after the fourth birthday, OR any 4 doses. (Age 7 or older): 3 doses.

Haemophilus Influenza B(Hib): one dose given on or after the first birthday for pre-school and pre-kindergarten students only.

Pneumococcal– one dose given on or after the first birthday for pre-school and pre-kindergarten students only.

Measles – one dose needed for any child entering pre-school or pre-kindergarten. 2 doses needed for any child entering kindergarten and older. Laboratory evidence of immunity is acceptable.

Rubella – one dose administered on or after the first birthday.

Mumps – one dose administered on or after the first birthday.

Hepatitis B – (Kindergarten or older): 3 doses Hepatitis B vaccine. If a child is between 11 to 15 years of age and has not received 3 prior doses of Hepatitis B vaccine, then the child is eligible to receive 2 doses of Hepatitis B vaccine at adolescent formulation.

Varicella – one dose administered on or after the first birthday. Laboratory evidence of immunity, physician’s statement or a parental statement of previous varicella disease is acceptable.

Meningococcal – One dose is required for children entering Grade 6 and all students transferring into a New Jersey school on or after 9-1-08 and born after 1-1-97.

Influenza-(age 6months-59months) 1 dose annually given between September 1st and December 31st of each year. Students entering school after December 31st up until March 31st must receive one dose since it is still considered flu season.

Mantoux Tuberculin Test – All students transferring between school districts in New Jersey, and from within the United States, will be exempt from testing. Students entering a New Jersey school district from a low TB incidence country are exempt from testing. Parents must check with the Nurse to determine if their child must be tested.
